A Comparative Studies of the Tubeless Gastric Analysis to the Intubation Method in Cases of Stomach Cancer and Poptic Ulcer

Abstract
The tubeless test for gasrtic acidity determination has been accepted as a simple, quick and
convenient method to detect free hydrochloric ace in the stomach since Segal and his associates
first reported a simple test using cation exchange resis in the quininium form in 1950, and later
developed a simpler and more accurate method with Azur A dye, quinine tetrabromophenolphthalein
or pyridine dye.
However, the tubeless method provides only qualitively and there are considerable differences
between the results of the tubeless and intubation methods especially in the case of negative results
of tubeless determination.
For the purpose of determinig the errors of tubeless method, both tubeless(with pyridine dye)
and intubation methods (with ethyl alcohol stimulation) carried out in each patient of stomach
cancers (14 cases) and of peptic ulcers(22 cases) who were admitted to Taegu Fatima Hospital
from October 1969 to May 1970.
The results were as follows.
1) When the results of tubleless test in the cases of either peptic ulcer or stomach cancer were
positive, it was considered that the result had reliable significance with almost 100% accuracy.
2) When the result of tubeless analysis were negative, no acid, its accuracy was 44% in the cases
of stomach cancers and only 10% in the peptic ulcers.
3) Because of considerable inaccuracy of the tubeless method when the results are negative, and
false negative which may be caused in various circumstances, in addition to cumbersome sampling
of urine, further improvement of this test is considered to be inevitable. Therefore clinically the
gastric juice analysis is selectively indicated, it is believed that the intubation method is rather
reasonablly acceptable than the tubeless method from the beginning.
4) Considering the significant reliability of positive results of the tubeless method in addition to
its simplicity, this tubeless method is thoght to have considerable benefits and on mass screening
for stomach cancer in the old age group.
